Dictionary is: "A printed or typed document with blank spaces for insertion of required or requested information". Originally forms looked like the one on the right. These paper froms still exist. They have boxes to fill in. When a form is completed it is submitted by hand or by mail. But on a web page, a form looks like a set of rectangular boxes where information is typed in. The boxes are called fields. There are push buttons at the bottom of a form. One button sends the information that was filled in.
